Change of Contact Details

It is essential that we are notified of changes to your address, telephone or mobile number. There are two ways of notifying us of any changes:

If you move out of the practice area you should not assume the Practice can continue to look after you. You will need to find a doctor in your new area.  There are instructions when you move to a new area on the back of your NHS medical card.
