Registration and Admin: To register as a patient ask at reception for details. You will be given a questionnaire to complete about your health and lifestyle, please take the time to fill this out carefully as it provides us with useful information. You will be officially registered with one particular doctor but you may choose to see a different one if you wish. We encourage you to stick to seeing the same doctor where possible. If you would like to give us feedback on the service we provide this is always gratefully received, you can either let your own doctor know or write a note to the practice manager. From time to time we will ask you to fill out feedback questionnaires, these are anonymous. The results of these will be posted on the news page, follow the link to the left of your screen. Should you have a complaint please refer to our complaints procedure which can be downloaded here. top Appointments: Please phone or come in to speak to a receptionist, we will endeavour to get you an appointment with the doctor of your choice, but please be aware this is not always possible especially at short notice. We will always see you on the same day if you have an urgent problem. top Visits: If you are unable to come to the surgery and would like a doctor to call, please try and phone before 10am. The doctors do their house calls between 12 and 3. It is always preferable for us to see you in the surgery so please do not ask for a visit unless it is really necessary. if the doctor feels that a visit is unnecessary they may phone and ask you to come into the surgery instead. top Opening Hours: We are open from 8.15 am to 6.30 pm Mon - Fri Morning surgery runs from 8.30- 10.30. We then see emergencies from 11.20 -12 Afternoon surgery runs from 3-6. One doctor is on call and will see emergencies between 5 and 6.30. As of 1/7/09 we will be offering extended opening hours. These will be 6.30-7.30 pm on Tuesday and 7.30 - 8.00 am on Wednesday, Thursday and Friday. Please note these appointments are to provide a service to patients who find it difficult to attend in normal working hours. Please do not book these appointments if you can attend during normal surgery opening hours. top Out of Hours services: On weekdays between 6.30 pm and 8.00am, all day and night at weekends and on bank holidays, provision of primary care services is the responsibility of Swindon primary care trust. If you phone the surgery during these times you will have a choice of speaking to or seeing an emergency doctor based at the Clover centre at the Great Western Hospital, alternatively you can be put through to NHS direct for advice from a senior nurse. top Surgery Timetable: Please note this is a provisional timetable and will be subject to change depending on other commitments.
